Getting his hand stuck in the grate was unintentional, but he left his hand in there intentionally. He did this because he saw this as an opportunity to end his life.
2. Do you think there could've been an ulterior motive to Chief smothering McMurphy?
No, I think the sole motive was to put McMurphy out of his misery because Big Nurse had turned him into a vegetable.
3. Does being at the mental ward actually make McMurphy crazy?
Not until the electroshock, then he starts to lose his mind.
4. Who is more to blame, the Big Nurse or Billy, for his death?
The Big Nurse because she shouldn't have put suicidal pressure on Billy by threatening to tell his mother.
